The Project

New Feature: Intake Form

The intake form is used to collect essential contact information from animal owners and detailed animal information to help veterinary staff prepare for appointments, ensure accurate medical records, and streamline onboarding.

Design Process

1. Identifying the Need
Veterinary clinics consistently requested a way for animal owners to submit their contact and pet information before visits.

This feature was one of the most frequently mentioned in:

  • Conversations with veterinary staff
  • UserVoice requests
  • Customer support feedback

2. Prototypes & User Testing
Using Lovable.dev, we created 2 low-fidelity prototypes to explore basic form structure and flow. We A/B tested prototypes to gather first impressions and make a determination of which prototype was deemed more intuitive.

“I liked version B better because it felt a lot less overwhelming from the start. The layout was cleaner and easier to follow, and it naturally walked me through the steps without feeling cluttered or confusing.”

3. Applying the Design System
The next step was translating the prototype into a high-fidelity, developer-ready design using Figma and our established design system.
